    Welcome to Little Achievers Preschool. Please be so kind to fill in this enrolment form with as much details as possible.





      An initial non-refundable registration fee of R 400.00 is payable on enrolment per child and in order to reserve a space in addition to a registration fee, one month’s school fees must be paid.

      PAYMENT OF FEES
      Yearly fees are calculated over eleven (11) months. Fees should be paid on or before the 1st day of every month in advance (e.g., if your child is starting school on the 1st of February, the fees for February must be paid on or before 01 of February). If payment is not received by the 3rd day of the month, a reminder will be issued.
      Furthermore, if payments have not been received by the 7th day of the month, a child or children will not be allowed to attend any further classes until all outstanding payments have been received with an additional penalty of R50.
      In cases which child/children whom, for whatever reason, is unable to attend school (e.g., hospitalization, visit to grandparents, overseas trips, etc.), the parents are still liable for school fees. No exceptions will be considered in this regard.

      DISCOUNT
      One month’s discount will be granted if yearly fees are paid by the end of March of each year.

      18 Months to 3 Yrs. R2 500.00
      3 to 6 Yrs. R3000.00
      Note: Grade R only is required to pay an additional R450 once off payment for workbook.

      EXTRA-MURAL ACTIVITIES

      The following extra-mural activities are offered and are included in your school fees:

      a) Swimming Lessons
      b) Kids Sport
      c) Chess Lessons
      d) Computers Lessons
      e) Birthday Celebrations
      f) Park Visit
      g) Photos
      h) Graduation Ceremony



      EMERGENCIES AND HEALTH

      If your child has any of the following sicknesses, please keep him/her at home:
      a) Fever: this can lead to fever convulsions so we feel it is best the child be kept at home or take to the doctor as this could be a sign of contagious sickness like chickenpox.
      b) Running tummy: this is also contagious and it is highly recommended to have it checked.
      c) Measles, chickenpox, German measles, mumps and any other concerning sicknesses that as a parent you would deem to need medical attention.
      The teachers should be informed if the child will not be coming in to school due to sickness by 09h00 in the morning. The school reserves the right to, in the case of an emergency; take your child to any other doctor who may be available at the time of emergency, if the child’s usual doctor as indicated in the application form is unavailable. The parents shall in such cases support the actions of the school and undertake to ascertain the circumstances surrounding the emergency from the school. Costs for which the parents are liable and are payable within 48 (Forty-Eight) hours following the incident.

      MEDICINE

      In cases where a child is on medication, the parents must ensure that the school is provided with the correct medicines and that clear instructions are provided as to the administration thereof. The parents must ensure that the medicines are clearly marked with the child’s name. The message book makes provisions for the following details to be included:

      a) The dosage to be administered
      b) Signature of the parent
      c) School representative signature

      If parent’s signature is not present in the message book, no medication will be administered. Should a child have physical abnormalities or deformities a doctor’s certificate specifying the nature of abnormalities must be presented to the school.

      Should a child arrive at the school with any injuries or visible lesions, the school reserves the right to request an explanation from the parent/s regarding the cause of such injuries. Should the parent/s refuse to provide this information, the school reserves the right to report the matter to the South African Police Service (SAPS) and/or the Department of Health and Welfare.

      a) It will be expected of parents whose children will be absent on a particular day, to inform the school accordingly, before 09H00 in the morning.
      b) Children with contagious sickness shall be kept at home in accordance with Health, and any other similar regulations.
      c) Should the child be allergic to any foods or liquids, the parent/s are to provide suitable alternatives.

      Part 4
      PERSONAL ITEMS

      All children’s clothes, bags, cases, bottles, blankets etc. must be clearly marked and is not the responsibility of the school if the item/s gets lost. The school will not accept any responsibility for toys that are brought to school and are subsequently broken or stolen.

      DURATION OF THE AGREEMENT

      This agreement is valid until a child obtain an age required in terms of South African Department of Education to be eligible to attend grade one (1).

      TERMINATION OF THIS AGREEMENT

      The agreement can be terminated by either party involved by giving a written notice of one (1) calendar month.

      The school undertakes to take all reasonable steps to prevent accidents or injuries to the children. The school will under no circumstances be held responsible for any sickness or injuries the child may incur whilst under the supervision of the school irrespective of the circumstances prevailing or due to circumstances beyond the school’s control.
